New Patient
Welcome, new patients, to our therapy practice! To make your visit as efficient as possible, please complete the new patient documents prior to your appointment. We offer a variety of payment plans and accept most insurances. Self-pay rates are also available. Please feel free to contact us with any questions. Our grievance policy is available upon request. We look forward to seeing you soon.
New Patient Documents
As a new patient, we want to make your first appointment as comfortable and stress-free as possible. You can print and fill out the new patient documents before your first appointment or arrive 15 minutes early to complete them in our office. You can also complete the documents via the client portal. Our team is here to answer any questions you may have and to ensure that your experience with us is a positive one.
Release of Information
At DeCoteau Trauma Informed Care and Practice, PLLC, we are committed to protecting your privacy and confidentiality. We understand that there may be times when you need us to release information to other professionals or agencies involved in your care. We want to assure you that we take this responsibility seriously and will only release information with your written consent.
Grievances
If I have a grievance about my provider, I understand that I am strongly encouraged to seek resolution of disputes through direct communication with the individual(s) involved before initiating the grievance process.

If the discussion does not resolve the matter, or you are not comfortable addressing the issues. Please file a written grievance within 5 working days of the final discussion.
